Hello!  Thank you for applying to be my Senior dress model.  You are applying for a photo shoot to be considered for publication in Senior Muse magazine. If I use more than one model their editors will decide who is featured in the magazine.  This photo shoot is free of charge to you.  Model(s) will receive their 3 favorite images free of charge with more digitals available for purchase. 

Fill out this form and send back to me asap.  Please don't be intimidated by any of the questions.  We can always work around a few things.  I just want to get a good idea of who you are and what you have going on this summer.    

I will choose my model(s) in April.  If you are chosen I will need to have a signed model release from you before the photo shoot.  If you are under 18 I will need your parent(s) signature.
